Fertility Forms

	Female Fertility Questionnaire – for new fertility patients. Please download and complete this questionnaire and return to the clinic by email or post ASAP. You will also need to complete a DASS and Mood and Stress Questionnaire as well.
	Male Fertility Questionnaire – for new fertility patients. Please download and complete this questionnaire and return to the clinic by email or post ASAP. You will also need to complete a DASS and Mood and Stress Questionnaire as well.
	DASS – for new fertility patients. Both males and females must complete a copy of the DASS questionnaire and return with other paperwork to the clinic.
	Mood and Stress Questionnaire – for new fertility patients. Both males and females must complete a copy of the Mood and Stress Questionnaire and return with other paperwork to the clinic.
	IVF Treatment Protocol – for fertility patients that are also using ART. Please download and read this information. Prior to commencing an ART cycle, you must contact the clinic as early as possible to arrange the recommended appointments.
